Collaborative Care


If you feel that worries, stress, or certain habits are preventing you from feeling healthy and happy, you are not alone. We offer behavioral health services through our Collaborative Care program to help you manage emotional, physical, and behavioral challenges.

The Collaborative Care Specialist is a licensed mental health professional with special training to help you become your healthiest self.

How do I get started?

You can choose to join the Collaborative Care program, or your family doctor or nurse may recommend you for the program. The Collaborative Care Specialist will work with you and your family doctor to create a treatment plan. You can see the Collaborative Care Specialist the same day that you see your family doctor.

Does my insurance cover this?

This service will be billed to your insurance carrier and is generally covered (copays, coinsurances, and deductibles will apply). If you have further questions, please call your insurance carrier or speak with a staff member.
