Community Health Centers, like Primary Health Network, are at the cornerstone of breaking down barriers associated with community needs and patient care. Transportation is an important social determinant of health in the communities we serve, and in order to address this, we have developed various programs that increase access to safe and convenient transportation options. Provide the Ride patient transportation program offers free door-to-door transportation to appointments for Primary Health Network patients who have no other means of transportation.

“Provide the Ride” has been instrumental in building access to care in PHN’s service areas. Existing patients are able to keep their appointments, which increases continuity of care, and new patients are able to receive preventive and/or early treatment.

Patient transportation services are currently offered in Beaver, Clarion, Mifflin, Erie, Lawrence, and Mercer counties. For more information or to schedule a ride, please call your preferred Primary Health Network location.
