DentistryGeneral Information:At The Primary Health Network, our nurses and receptionists work diligently to keep your waiting time as short as possible. You can help keep things running smoothly by remembering the following: FOR YOUR FIRST VISIT Arrive 15 minutes early to allow time to complete paperwork. Bring a companion if you need help to complete paperwork. Name and address of previous or referring physician. List of all present medications or bring bottles with you. Current insurance cards. Proof of income, if no insurance . Sliding fee billing available if eligible. FOR EVERY VISIT Parent or Guardian must accompany children under age 18. Current insurance cards. Payment or Co-payment is expected at time of service; we will bill insurance as a courtesy. Proof of income, if no insurance. Bring/request referrals as necessary. If you need a referral, please request it at least 48 hours prior to the appointment. Bring someone to supervise children in the waiting area, do not leave them unattended. Appointments are rescheduled after 15 minutes late. If you must cancel or reschedule, please call 24 hours ahead.
