Dr. Louis Mastrian provides optometry services at Farrell Health Shoppe, and at Buhl Farm Community Health Center. He has been affiliated with PHN since 1997. Dr. Mastrian earned his optometry degree from the University of Missouri, St. Louis. He also completed an externship where training focused on vision rehabilitation, contact lens care, ocular pathology, vision therapy and primary care.

Primary Health Network accepts most major insurances including Medicare, Medicare Advantage Plans, Medicaid, Medicaid Managed Cares, Commercial Insurance, Private Insurance, W/C, and Auto Accident Insurance.
